Sure, it's certainly doable using the Acrobat SDK (ie. as a plugin).

It's not a single API call - you need to do a bit of work.
You'll first need to iterate over each page, then iterate over the
annotations on each page. For each annotation, you need to find out
whether it has an action associated with it. If it does, then is the
action a URI action? If it is, then you get the URI for the action
and if you wish to change it - you do!
