Jami,
Take a look at Amgraf's OneForm Designer Plus' PDF Generator Option, (www.amgraf.com/pages/iforms.html). This allows data to save/retrieve to the server DB using only Acrobat Reader. Data can also be interchangable w/ HTML version form for local save if needed. Contact me if you need details.
