I have a Canon DR4050 scanner. Not cheap, but it will produce an Image PDF. It can also OCR based on Zones that you define (Text, Bar code, etc). It will take this OCR data and save the file under the name of the OCR data. It also supports Separator Pages so you can load several hundred pages of identical documents (I use it to scan Medical Records), and each document will have a unique filename based on the OCR data you have defined.
