I found a solution as PDF Maker does not appear to work correctly.
Use ABSOLUTE pathnames in the Word doc.
For example:
don't use ../dir_B/fname_B1.pdf
do use http://www.domain.com/dir_B/fname_B1.pdf and then use Convert to PDF.
