Topic: custom appearance annotations with ? over the top

I have a pdf that has custom appearance annotations, in reader 5.1/5.5 it looks fine, but in Adobe Reader 6 most of the custom annots view ok but one has a question mark in a gray box over the top of the annotation (underneath is ok), any ideas what the cause of this might be or what this means ?
