I can see a way of setting the hyperlink appearance manually in Acrobat:
1. Open PDF for changes.
2. Change to text selection tool, click on first character in document.
2. Find the first hyperlink in the document by View | Navigation Tab Tags | Content | Options | Find... | Unmarked Links ... Search Document.
3. Select Link Tool.
4. Right-click on hyperlink.
5. Right-click hyperlink, Edit | Select All. All the hyperlinks in the document are selected.
6. Right-click first hyperlink, Properties... Set appearance properties, select Locked if appropriate. Close dialog.
All the hyperlinks receive the properties. Click the Hand Tool, and browse the document to see the result.
