Topic: Text Space/Shrink To Fit/Select Printer

I am new to PDF development but unfortunately I am under the gun. I have spent the day reading various documents but have not been able to find answers to some questions I have.
1) I don't have a firm grasp on the text space. It would make sense that if you specify the coordinates of some text, that the entire text would be contained within those coordinates. This does not seem to be the case, however. For example,

1 0 0 1 288 756 Tm
This does not seem to print out with the top of the text at 756, but rather seems to print out with the middle of the text at that location. So I guess my question is, what does the Text Space really define?
2) I have set my page MediaBox to [0 0 612 792], which should be the size of standards letter paper (in the US), but when I have 'shrink to fit' selected in Acrobat Reader, it shrinks the page. Why would it do this if the document size is defined properly?
3) I guess this is really more of an Acrobat question. I have the user select which printer they would like to use in my VB app. I would like my app to change the printer that Acrobat Reader uses to match what the user selected. I don't want to change the Windows default printer as this would affect other apps and I can't use PrintFileTo because I want to display the document in AR first and let the user print it if they so choose. Is there any way to do this?
