Question
Is there software available that can handle HTML form data over the web,
combine this data with a PDF file contained within an online library of PDFs
and present a new PDF 'image' over the web to the user with the new form
data.

In other words, a web visitor fills-in an online HTML form, calling a
specific PDF from a library, combines that data in graphical format creating
an appended PDF from this library and presents the new PDF with that data
for web viewing and download as a new file.
