The specs:
Original PDF File: Capture Reviewer 2.01, PDFWriter 3.03, both on WinNT
PDF Template: Acrobat 5.0.5 on Win2k

I've been trying to put a navigation template to all pages of a PDF, based on this trick posted here:

http://www.planetpdf.com/mainpage.asp?MenuID=150&WebPageID=346

But it's not working at all. The template isn't propagating on any pages. I also noticed that the bookmarks in the original PDF file aren't working either, if that is of any help in any way.

Is there any variation of the tip posted at the link above that can assist me, or is there some other way to programmatically put this template on all the pages of the PDF file? The original PDF File is over 400 pages, and we have a number of these large files created via capture reviewer, and I'd really rather NOT do this by hand.
