Custom stamps in Acrobat 5 were a big hassle to setup. It included having to insert pages into a PDF, setting page templates, very annoying. They are now a *LOT* easier in Acrobat 6.
In Acrobat 6, you can create your custom stamp quickly and easily from the interface. 'Tools > Commenting > Stamp Tool' will take you where you need to be, then you can just choose to create a new custom stamp, select your image/pdf file to stamp, and off you go!
