I used Act! and Outlook and auto-fill their data into Acrobat forms:

Your setup is under 3 minutes and you can reload the setup for future reuse - in seconds. Upon setup, click a button and simply wait for your forms to be auto-fill/email/fax. Amazing stuff i'll tell you.
